If accessing this form on an iOs (i.e. Apple) device make sure to open it in the Safari browser, all payment options are not available if the form is opened in other browsers. Make sure to fill in all required fields, including acknowledgments for the Waiver, and Codes of Conduct. Select the sport being signed up for from the Products list. The CFCA dues will be automatically selected in the list, as the annual dues cycle starts each Spring. Dues are per family, so any subsequent signups after the first child the Dues box can be unchecked. The total cost (including Electronic Payment Convenience Fee) will be calculated, and you can choose Credit Card or PayPal payment option. PLEASE BE AWARE, IF CLICKING THE PAYPAL OPTION, A VENMO OPTION WILL ALSO BE DISPLAYED. DO NOT CHOOSE THIS OPTION AS THERE IS A KNOWN BUG WITH VENMO AT THE CURRENT TIME AND PAYMENT WILL NOT PROCESS. You will receive a confirmation email for your signup at the email address you provide.

Forms and instructions

You will need to download and print out the forms below. You can either mail the completed forms or bring them to the sign-ups on June 11 or 12 at the C.F. High School cafeteria from 6 to 8 p.m.

Step 1: Download the sign-up form

Step 2: Fill Out the Form

If you mail the forms in, please send an email to cfcaquestions@gmail.com notifying us you will be mailing forms. Completed forms can also be brought to in-person sign-ups.

Step 3: Payment options

Venmo app (search for charities, Chenango Forks Community Association, Inc., CFCA-1)
PayPal (search for cfcaquestions@gmail.com)
Check payable to CFCA

Payment notes:

Be sure to add the $1.50 processing fee to the total fee for all sports and dues.
Add a comment to the transaction to indicate child's name and sport being signed up for: e.g. “Jimmy Smith – Baseball”
Click on the "Donate" button to submit. (Since CFCA is a 501c organization the app displays the button as "Donate" instead of "Pay")
If mailing or emailing forms, please indicate "Paid with Venmo" or "Paid by PayPal" on the signup form.
If you include an e-mail address on your form, you will receive e-mail notification of receipt of your forms and payment.

All Spring Sports sign-ups should include $5 dues per family.
