Sign-up forms and instructions

You will need to download and print out the forms below. You can either mail the completed forms or bring them to the sign-ups on February 13 or 15 at the C.F. Hhigh School cafeteria from 6 to 8 p.m.

If you mail the forms in, please send an email to cfcaquestions@gmail.com . If you have any questions about this process, or experience difficulty accessing the forms, please send an email to cfcaquestions@gmail.com .

Step 1: Download the sign-up form

Download the CFCA Sign-up form (.pdf, 49kb).

Step 2: Fill Out the Form

If you mail the forms in, please send an email to cfcaquestions@gmail.com notifying us you will be mailing forms. Completed forms can also be brought to in-person sign-ups.

Step 3: Include Payment

Payments may be made via the Venmo app (or Search for charities, Chenango Forks Community Association, Inc., CFCA-1), for a $1.50 processing fee. .

• Be sure to add the $1.50 processing fee to the total fee for all sports and dues you are paying for.
• Please add a comment to the transaction to indicate child's name and sport being signed up for: e.g. “Jimmy Smith – Baseball”
• Click on the "Donate" button to submit. (Since CFCA is a 501c organization the app displays the button as "Donate" instead of "Pay")
• If mailing or emailing forms, please indicate "Paid with Venmo" on the signup form.

Electronic Payment to the CFCA
The CFCA recently enabled the receipt of electronic payments for sports fees via the Venmo app. Users of this capability will need to add $1.50 to their total fee to cover app processing fees. Find instructions for using electronic payment on the "Sign-up by Mail" page. Any questions regarding electronic payment should be sent to cfcaquestions@gmail.com .

Free/Reduced Lunch Program: If your family is experiencing financial hardship please email cfcaquestions@gmail.com regarding reduced fee options.

Make your check payable to CFCA If you include an e-mail address on your form, you will receive e-mail notification of receipt of your forms and payment.
