Sign-up forms and instructions

You will need to download and print out the forms below. You can either mail the completed forms or bring them to the sign-ups on October 4th at the C.F. Middle School cafeteria from 6 - 8:00 p.m. The deadline to sign up for Boys and Girls programs is Friday, October 27th. FBA sign-ups will be accepted through December.

If you mail the forms in, please send an email to CFCA@stny.rr.com. If you have any questions about this process, or experience difficulty accessing the forms, please send an email to CFCA@stny.rr.com.

Step 1: Download the sign-up form

Download the CFCA Sign-up form (.pdf, 49kb).

Step 2: Fill Out the Form

If you mail the forms in, please send an email to CFCA@stny.rr.com notifying us you will be mailing forms. Completed forms can also be brought to in-person sign-ups.

Step 3: Include Payment

• FBA (co-ed, grades K-2) $25
• Boys Basketball (Grades 3-6) $60
• Girls Basketball (Grades 3-6) $65

Free/Reduced Lunch Program: If your family is experiencing financial hardship please email cfca@stny.rr.com regarding reduced fee options.

Make your check payable to CFCA If you include an e-mail address on your form, you will receive e-mail notification of receipt of your forms and payment.
