Winter Sports Information
Sign-up forms and instructions
You will need to download and print out the forms below. You can either mail the completed forms or bring them to the sign-ups on June 11 or 12 at the C.F. High School cafeteria from 6 to 8 p.m.

Step 1: Download the sign-up form
Download the CFCA Sign-up form (.pdf, 49kb).
Step 2: Fill Out the Form
If you mail the forms in, please send an email to cfcaquestions@gmail.com notifying us you will be mailing forms. Completed forms can also be brought to in-person sign-ups.
Step 3: Payment options
Payment notes:
- Be sure to add the $1.50 processing fee to the total fee for all sports and dues.
- Add a comment to the transaction to indicate child's name and sport being signed up for: e.g. “Jimmy Smith – Baseball”
- Click on the "Donate" button to submit. (Since CFCA is a 501c organization the app displays the button as "Donate" instead of "Pay")
- If mailing or emailing forms, please indicate "Paid with Venmo" or "Paid by PayPal" on the signup form.
- If you include an e-mail address on your form, you will receive e-mail notification of receipt of your forms and payment.
Costs:
- FBA (co-ed, grades K-2) $25
- Boys Basketball (Grades 3-6) $60
- Girls Basketball (Grades 3-6) $65
